At CCPC, we seek to provide caring and holistic case management. This is a brief description of what we provide and our case management protocol.
Case Management Services
Holistic Needs Assessment
We seek to identify strengths and abilities, needs, preferences, family and natural supports, and goals that promote wellness-based outcomes through this assessment.
Assist Individuals
We help individuals obtain basic needs and identified services through collaboration with community service providers to address specific individual needs and attain specified goals.
We provide assistance and hope to individuals and their families through coordination of care with other supports and networks.
Assist Families
We provide assistance and hope to individuals and their families through coordination of care with other supports and networks.
Education and Planning
We assist individuals and families in their efforts toward achieving self-sufficiency through education, addressing barriers, goal planning and accountability.

Basic Needs Assistance
The focus of Basic Needs Assistance is to assist families who are in need of food, clothing and/or shelter. We are about to assist with the immediate needs of food and clothing and can offer hygiene kits, and shower and gas vouchers.
We can also offer resources for shelter and — if we have funds — we may be able to offer rental or utility assistance, or a night stay at a motel. Any financial assistance may take up to one week for approval. Funds are never offered the day they are requested and they are never distributed directly to the individual requesting assistance.
The case manager works collaboratively with other community providers to proved wrap-around services and resources to meet people’s needs.
